It is widely used for acute ischaemic stroke in Russia, Eastern Europe, China and other countries, and the FDA has not approved it. Available from 8 vendors, priced from $49.99 to $119.99.
Research: The 2023 Cochrane review covered seven randomised trials with 1,773 participants (Ziganshina et al, Cochrane Database Syst Rev 2023;10:CD007026, PMID 37818733). It found that Cerebrolysin or similar peptide mixtures probably make little or no difference to death after acute ischaemic stroke (risk ratio 0.96, 95% CI 0.65 to 1.41). It also found they probably increase non-fatal serious adverse events (risk ratio 2.39, 95% CI 1.10 to 5.23). Both are moderate-certainty findings, and the manufacturer supported three of the included trials. The largest positive trial, CARS, reported better arm function at day 90 with 30 mL daily for 21 days (Mann-Whitney estimator 0.71, 95% CI 0.63 to 0.79; Muresanu et al, Stroke 2016;47(1):151-159, PMID 26564102). Company employees are among its authors.
Side effects: Cochrane 2023: more people with non-fatal serious adverse events than on placebo (risk ratio 2.39), more marked on the 30 mL for 10 days schedule (risk ratio 2.87), no difference in the total number of people with adverse events (low-certainty evidence)
