Research: STEP 1 (Wilding et al, NEJM 2021, PMID 33567185) randomised 1,961 adults and reported a 14.9% mean weight change at 68 weeks against 2.4% on placebo, on the treatment-policy estimand, with 86.4% of the treated group reaching at least 5% loss. SELECT (Lincoff et al, NEJM 2023, PMID 37952131) enrolled 17,604 people aged 45 or over with established cardiovascular disease and a BMI of 27 or over, and no history of diabetes. Major adverse cardiovascular events occurred in 6.5% against 8.0% on placebo, a hazard ratio of 0.80 (95% CI 0.72 to 0.90). That is a 20% RELATIVE reduction and about 1.5 percentage points in absolute terms.
